BF Lorenzetti has instituted a new process for applying for insurance for special events.  Please check our forms page to use the application.  I have linked the application website to the first page of the step by step instructions.

This application is to be used for all extra curricular activities such as dry land training, facility rental etc.  Should you get a request for proof of insurance from your municipality or other facility you use this form to obtain your proof of insurance.
